Personal Health Report
Advanced online technology provides a platform for personalization of each question set to the user, as well as providing individualized guidance for their unique set of risks and readiness.
Upon completion of the Mayo Clinic Health Assessment, individuals receive a comprehensive and confidential Personal Health Report outlining their health status and risks. A personalized report provides positive feedback and reinforcement on health strengths as well as direction for addressing health risks based on their readiness to change. The Personal Health Report includes:
- An easy-to-understand Results Summary outlining the user’s risk level (low, moderate, high or very high)
- A comparison of individual risk results to recommended health scores, and a look at their health status compared to the last time they completed the Health Assessment
- A detailed report on each risk measured, including an explanation of the risk and actionable information on what can be done about it
- Links within the Personal Health Report guide users to information and tools within the custom Mayo Clinic site to address their specific health risks, and to company-specific resources that address each risk identified
- Company-specific links and health messages can be customized both within the text of the Personal Health Report, as well as in a highly visible right-hand column that allows for visuals and longer text to accompany a link.
